I recently finished up a program called Exercise is Medicine at a Mayo System hospital here in MN. There were people there in really bad shape, moderately bad, and some in fairly good shape who were rehabbing an injury. The big thing that I took away from the program is that consistency, not intensity should be the goal of exercise. I see people doing outlandish things in order to hit their 4% but I wonder how long they will be running for 2 hours in Death Valley or whatever.
